Please Complete the below questionnaire prior to your Squads ice-time on the same day.
REMINDER- THIS FORM IS ONLY VALID IF COMPLETED ON THE DAY OF YOUR ACTIVITY.
A new form must be completed prior to each entrance of the facility.
 
AMRA has moved to Online screening for the remainder of the season. Please Complete the below questionnaire prior to your Squads ice-time on the same day.

All Players, Bench Staff and Parents attending facilities are required to complete this DAILY COVID-19 ATTESTATION AND AGREEMENT. All Players are required to complete a 48-HOUR RSVP, which will be sent to you through Goalline Attendance. The onus is on the parent and/or participant to ensure that they have completed the necessary forms.
